The table should have fields separated by commas and NO quotes around the fields.
The first row in the table needs to be the database column names.
Select Table for matching:

Select CSV File on your desktop:
Or enter a filename on the web server:
A merges CSV file will be output. Or check here
to update database file.
If you select "Update Database File", unmatched records are written to a csv file. Or check here
to add them to the database file.
The file is assumed to be coma delimited. Check here
for TAB delimited.
